The Department of Commerce (through legislation passed recently) has made
the process for minor work permits more standardized. The forms can now
be printed off their website and simply turned into the school district in
which you reside.

You will need to print off the pre-application form. This has a section
for the parents to fill out and for the employer to fill out. You will
also need to print off the physician's certificate. Once
you have completed these
forms, along with proof of home education (a copy of your notification
cover page) must be turned into the school district. They will then file
everything with the Dept. of Commerce.
