  (Required for ALL graduates)

$65.00 includes gown, cap, tassel and shipping & handling charges.  You will receive the cap and gown approximately 2 weeks before graduation via insured US Postal Service.  The caps and gowns, in keeping with CHEO's colors, will be burgundy, gray and white. We will NOT be able to exchange sizes this year!

Write a narrative about the graduate (50 words or less). List the awards, honors, activities and future plans after graduation. This will be read by the MC during the graduation ceremony. We reserve the right to edit the narrative.

Review your information carefully for proper spelling, correct measurements etc.; after you submit the form, you cannot make changes to the information.  Click "submit" when you are sure your form is complete.

You may either pay on-line with a credit card through our website or mail in a check. If you wish to use a check please write a check (payable to CHEO) for $65.00 by March 1. Mail your check to:
